• Information shared during your clinical assessment (e.g. substance use history, mental health concerns, treatment preferences)  
• Notes, forms, and feedback relevant to your rehab referral process  

We only collect what’s necessary to provide safe, accurate, and effective service.️ 

We use your information to:

• Conduct a personalised addiction assessment  
• Prepare a de-identified case summary for potential rehab providers  
• Provide referral options and guidance  
• Communicate with you regarding your service, bookings, or support  

We never sell, trade, or rent your personal data. Ever.

 When we share your information with rehab centres, we first remove all names, contact info, and identifying details. The summary includes only what’s medically and clinically relevant to help match you with suitable options.Only after you select a provider do we share your name and contact information directly with them — with your permission.

All personal data is:

• Stored securely using encrypted systems  
• Accessed only by authorised ClearPath Rehab team members  
• Retained only as long as necessary for your service or as required by law  

We comply with the Australian Privacy Principles (APPs) under the Privacy Act 1988 (Cth).

• Correct inaccuracies  
• Withdraw consent  
• Ask us to delete your data (unless legally required to retain it)
